Frequently Asked Questions

Why might I need follicle tracking?

You might benefit from follicle tracking if you are trying to conceive and want to time intercourse to your most fertile days, if your periods are irregular or unpredictable, or if you have been trying for a while without success. It is also widely used to monitor cycles during fertility treatment such as IUI and IVF, and to assess ovulation in women with PCOS or suspected anovulation.

Tracking takes the guesswork out of timing. Home ovulation kits and apps only estimate when you might ovulate. A scan actually sees your follicles grow and confirms when and if ovulation happens. The sooner you have that clarity, the sooner you can make confident decisions about conception, treatment or simply reassurance.

What is follicle tracking and what does it check?

Follicle tracking is a short series of ultrasound scans across your menstrual cycle that follow what is happening in your ovaries and womb in real time. Over the series, your sonographer looks at:

  • The number and size of developing follicles in each ovary.
  • Which follicle becomes dominant (the one most likely to release an egg).
  • How quickly the leading follicle is growing day by day.
  • Whether and when ovulation has taken place.
  • The thickness and pattern of the endometrium (womb lining).
  • Whether the lining is responding well and is ready for implantation.
  • Any ovarian cysts or features suggesting PCOS.

Together these give a precise, day-by-day picture of your cycle that no home test can match.

How does follicle tracking confirm ovulation?

As your cycle progresses, one follicle usually grows faster than the rest and becomes the dominant follicle. Tracking scans measure it across several visits, watching it enlarge until it is mature.

Ovulation is confirmed when the mature follicle is no longer seen on the next scan. It has released its egg and collapsed, often with a small amount of fluid behind the womb and a developing corpus luteum in its place. The womb lining also thickens into a receptive pattern over the same window.

Because the scan shows these changes directly, a follicle tracking scan confirms whether ovulation has genuinely happened, rather than simply predicting it.

How does a follicle tracking ultrasound work?

A follicle tracking ultrasound uses safe, pain-free sound waves, no radiation, no needles, no contrast injection. Each scan is performed using transvaginal ultrasound (TVS), which gives the close, high-resolution view needed to measure follicles accurately:

  • Transvaginal ultrasound (TVS), a slim, lubricated probe (about the size of a tampon) is placed in the vagina, giving a detailed view of both ovaries, each developing follicle and the womb lining.
  • Serial measurements, the same checks are repeated over several visits so your follicles can be measured as they grow, the dominant follicle identified, and ovulation confirmed.

Follicles are measured to the nearest millimetre and the womb lining is assessed at each visit, building a clear day-by-day timeline of your cycle.

Because ultrasound carries no health risks, you can feel comfortable and confident throughout.

What should I expect during my follicle tracking scan?

Please take the opportunity to ask any questions or raise any concerns, your experience is our number one priority. Each scan is carried out by a female Specialist Senior Sonographer unless you request otherwise, and you can pause or stop at any point.

Each visit is short and follows the same simple steps:

  • Empty your bladder, the transvaginal scan is best performed with an empty bladder, so you will be invited to use the bathroom on arrival.
  • Transvaginal scan, you will lie on your back with your knees bent and legs supported by a privacy sheet. A slim, lubricated probe is gently inserted into the vagina. The lubricating gel may feel cool at first but will soon warm to body temperature. The scan should not hurt, notify your sonographer if you feel any discomfort.
  • Measurement and review, your follicles and womb lining are measured and the findings explained on the screen in real time.

Most appointments take only ten to fifteen minutes, so each visit in the series is quick and easy to fit around your day.

How many follicle tracking scans will I need, and when?

Follicle tracking is a short series rather than a single scan. The exact number depends on your cycle, but most women need two to four scans across one cycle.

The first scan is usually performed from around day 10 of the cycle (counting day one as the first day of your period), when the dominant follicle is starting to stand out. Further scans are then spaced every one to three days as the follicle approaches maturity, with a final scan to confirm that ovulation has taken place.

If your cycles are irregular or you have PCOS, the timing is tailored to you and a few extra scans may be helpful. Your sonographer will advise exactly when to return after each visit so the series fits your individual cycle.

How should I prepare for the scan, and is it safe?

There is very little to do. Each transvaginal scan is best performed with an empty bladder, so simply use the bathroom when you arrive. There is no need to fast, and you can take any regular medication as normal. It helps to know the first day of your last period so the series can be timed correctly.

Follicle tracking ultrasound is extremely safe and pain-free. The process involves no radiation, no needles and no contrast, and you should not experience any side effects before, during or after each scan. You can return to your regular day-to-day activities, including driving, work and exercise, immediately following every appointment. A small amount of clear lubricating gel may remain afterwards, which is completely normal and clears on its own within a few hours.

What do follicle sizes and ovulation signs mean?

Follicles are measured in millimetres and grow steadily through the first half of the cycle. As a rough guide, a follicle usually becomes mature and ready to release an egg at around 18 to 24mm, often growing about 1 to 2mm per day in the days beforehand.

Alongside follicle size, your sonographer assesses the endometrium (womb lining), which thickens and takes on a characteristic three-line pattern as ovulation approaches, a sign the lining is becoming receptive to a pregnancy.

Once the mature follicle has gone on a later scan, with fluid behind the womb and a developing corpus luteum, this confirms ovulation has occurred. Every measurement is explained to you clearly so you understand exactly where you are in your cycle.

Can follicle tracking help with PCOS or absent ovulation?

Yes. Follicle tracking is particularly useful if you have polycystic ovary syndrome (PCOS) or suspect you are not ovulating regularly ( anovulation ). In PCOS the ovaries often show many small follicles, and ovulation can be irregular or absent, which makes timing conception by dates alone very unreliable.

By scanning across the cycle, tracking can show whether a dominant follicle is developing, whether it goes on to release an egg, and how the womb lining is responding. This gives you and your specialist a clear, objective picture rather than guesswork.

If you are using ovulation-induction medication, tracking is also used to check how your ovaries are responding and to reduce the risk of too many follicles maturing at once.

Can follicle tracking support IUI or IVF treatment?

Yes, follicle tracking is a core part of fertility-treatment monitoring. In an IUI cycle, tracking pinpoints the optimal day to perform the insemination by following the leading follicle until it is mature and ovulation is imminent.

In an IVF or ovulation-induction cycle, serial scans monitor how the ovaries respond to stimulation, counting and measuring the developing follicles, so the timing of trigger and egg collection can be planned and the risk of over-stimulation reduced. The womb lining is checked at the same time to confirm it is developing well.

The treatment I was given was excellent by Dr. Ahmed who explained everything thoroughly and ensured that I understood the procedure before going ahead with an injection in my arm.
